Mail Registration

Mail in and fax registration can be completed by simply filling out the Pacific Coast Men's Summit Registration Form. Click the "Registration button above to launch the PCMS Registration form (you will need Adobe Acrobat Reader to view this). Once it opens, you can either press the "print" button at the top or scroll down and fill out the form on your computer. Then use the print button to print out the completed form. Once completed, you can mail it with your check, money order or cashiers check to:

All registration will be processed on a first come, first serve basis. Lodging and special consideration for special needs will be based on first come basis as well. No registration will be processed unless payment is paid in full. We do not accept deposits to "hold" a position. Therefore, payment must be received before your registration will be processed.

Fax Registration

Alternately, you can fill out the form using the method above and fax it to (503) 842-3958. However, the registration will only be held for 4 days while we await your payment for the Summit. If you fax your order in, we can only accept check, money order or cashiers check to the above address.
